Preschool Forms and Information

Important information and forms for parents whose child(ren) are enrolling in Temple Har Zion’s preschool.

An application form must be completed for enrollment of each preschool student.

Once enrolled, please email the following to the Director of Early Childhood Education, Claudine Guralnick:

Headshot or closeup photo of your child (for labeling cubbies, chairs, art display, etc.)
Family photo for classroom display and school directory

A week before your child(ren)’s first day of preschool, you’ll receive an invitation to Remini and a text from our Emergency Communication System.

Please complete and return the following documents for each preschool student prior to starting preschool:

- Family Profile
- IL Certificate of Health Examination (DCFS required medical form). This health document must be dated within six months of school start date and signed by the child’s healthcare provider. Parents must fill out and sign the parent portion of the document.
- Receipt of DCFS Summary Guidelines
- Copy of Birth Certificate
